Direct Debit Terms and Conditions

  1. I authorize my banking institution to debit from the account indicated in the completed form, through a direct debit system (SDD), at the request of APCM, the amount(s) specified, in accordance with the defined periodicity.

  2. The amount indicated above will be debited from my account by the 15th of each month, except for the first debit, which may be processed during the month of registration.

  3. I am informed that debits may be made on different dates and that, in the event that a debit cannot be successfully processed, another attempt may be made during the respective month.

  4. The amounts and data provided may only be changed with my authorization.

  5. I will inform, in writing, both my banking institution and APCM, at least 10 days before the agreed debit date, if I wish to cancel the instructions indicated herein.

  6. I acknowledge that if any debit (SDD) does not comply with the instructions indicated herein, I will have 8 weeks to file a complaint with my banking institution and with APCM, which will refund the amounts in question.

  7. This payment method will not incur any charges for the new Friend.

  8. I expressly authorize APCM, directly or indirectly, to process the provided data electronically and to use it within the scope of its activities, as well as to cross-reference it for statistical purposes.

  9. I am aware of my right to access my personal data stored in the database and to request its update, deletion, or correction.

  10. I confirm that the information provided in this form corresponds to my true identity and signature, and has not been intentionally falsified.

  11. I acknowledge that, if requested, I will provide a signed copy of this information as proof of my authorization for regular donations to APCM.
